Ideal candidates will have experience with telephonic identification and coordination of needs in chronically ill patients and families, as well as providing clinical and administrative support to providers. Responsibilities will be highly varied, and appropriate candidates will be multitalented, flexible problem solvers who are eager to tackle complex problems and tasks.

A successful candidate will have an active RN lice nsure (required). Minimum of 3 years clinical experience required, preferably in a multidisciplinary team environment. Prior healthcare experience serving chronically ill patients (preferred). Fluent in Spanish (preferred, but not required). Telephonic case management or care coordination experience a plus.

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Strong customer-service orientation. Ability to be patient, compassionate and use a professional demeanor. Exceptional organizational skills and ability to multi-task. Self-starter with an entrepreneurial spirit and demonstrated problem solving skills. Strong writing, Word, Excel, and database management skills. Extremely strong work ethic. Must be able to work in a fast-growing, rapidly changing environment.

Telephonic care coordination with palliative care patients and their families, including incoming calls about acute issues as well as proacti ve outgoing calls to build rapport with patients telephonically while educating on disease process, conducting goals of care conversations, and addressing acute issues. Assess, identify and close clinical and non-clinical gaps in patient care

Appropriate triage and escalation of routine, urgent and emergent patient needs.

Clinical care coordination with PCP’s and other providers and specialists, including but not limited to:

  • Coordinate Aspire Nurse Practitioner home visit as needed
  • Coordinate with other providers regarding patient condition
  • Initiate clinical service orders on behalf of the patient
  • Update prescriptions and refill requests
  • Coordinate health plan authorization for Home Health, DME, Home Infusion and other services in conjunction with the Nursing Support Specialist .
  • Medication management, including identifying potential medication issues, answering patient questions on side effec ts and medication conflicts, and coordinating refills.
  • Coordination with Social Workers to help identify community resources and assist Aspire’s patients and families in accessing those resources.
  • Refer patient to Aspire Health social worker and chaplain resources when appropriate. Provide comfort and reassurance to patients and family members.
  • Participate in interdisciplinary team meetings as appropriate.
