  • Provides hands on care to each assigned patient. Supervises and teaches team members, includ! ing RN's, LPN's and HHA's in the efficient and effective delivery of quality in-home patient care.
Provides nursing care and treatment of home infusion patients in accordance with the physician's plan of treatment and with company policies and procedures, and all applicable accreditation, federal, state, and local regulations.
  • Collaborates with infusion pharmacy staff to develop and monitor goals of patient therapy.
DUTIES

  • Accepts patient care reports and patient assignments from the Team Coordinator for all patients assigned. Coordinates with the Team Coordinator timely completion of assigned visits and submission of the required documentation.
  • Provides frequent communication to the Team Coordinator, physicians, other team members, etc., and the patient/family in efforts to coordinate changes in patient care and assures appropriate follow-up and supporting documentation is done.
  • Assigns, develops, super! vises and evaluates personal care services provided by the sta! ff in accordance with organizational policies, procedures and processes and Medicare/State regulations.
  • Develops and revises, when indicated, the personal care-care plan for each patient receiving care.
  • Makes assigned patient care visits in accordance with physician orders. Completes supporting documentation in an accurate and timely manner, including but not limited to; visit report, OASIS assessment, care plan/plan of care revisions, medication profile, progress notes, revisions to the plan of treatment for recertification, discharge/transfer summary, etc.
  • Able to access and care for implanted ports independently; Independent in PIV placement and care; Independent in Midline or PICC placement and care; Proficiency with a variety of ambulatory infusion pumps; TPN patient management; Advanced Pain Management Skills (Demonstrated knowledge of intrathecal and epidural catheter care. Independent in access and programming of implantable pain pumps;
QUALIFICATIONS

  • Registered Nurse, currently licensed in the state of Tennessee.
  • Demonstrated competency in venous access device management, infection control, and safe practice of infusion nursing.
  • Current valid Tennessee driver's license.
  • Must be able to provide direct patient care
  • Working knowledge of the home health care program and previous experience providing home health visits preferred.
