The New Palliative Care Unit is holding an RN Job fair on Friday, July 20th and Wednesday, July 25th from 8 a.m.-4 p.m.

The Palliative Care Inpatient Medicine Unit is a 16-bed unit which is planned to open in August 2012. Initially, 9 rooms will be dedicated to patients of the Palliative Care Team. Some of these patients will be candidates for compassionate extubation from a ventilator. The staff nurse will be a key member of the team and work with the Palliative Care provider, patient, family and other members of the interdisciplinary team to develop and implement a well defined plan of care. Focus will be on effective symptom management and improving quality of life. Family/support individuals will have special support including bereavement resources. Approximate nurse to patient ratio will be 1 nurse to every 4 patients with variable staffing based on patient needs. Other patients on this unit will meet general medicine acute inpatient criteria.

Job requires a graduate from an accredited RN program and at least 1 year of experience in acute hospital. Prefer 2 years of recent acute care (hospital) nursing experience in past 5 years, ventilator experience in LTAC or critical care.

Upon graduation and obtaining a degree in Nursing from a NLN or CCNE accredited program and Tennessee state-approved program, applies the nursing process to plan and manage the care of patients, including patient and family education and continuum of care planning. Effectively communicates pertinent patient/family information to health care. team. Participates in finding solutions for variances from patient goals and ensures implementation of solutions. Furthers professional growth by engaging in 6- 10 hours of learning activities annually.

Key Functions and Expected Performances:
1.PLANNING & MANAGING CARE a.Practice adheres to specialty and organizational standards. b.Addresses patient safety needs c.Applies age specific/cultural considerations to care of patient. d.Organizes and triages patient care - timely, accurate, and acuity determination. e.Manages patient flow for maximum efficiency and notifies internal resources of changes f.Appropriately communicates changes in patient condition. g.Takes on additional clinical responsibilities and/or assists others. h.Takes on additional clinical responsibilities and/or assists others. i.Delegates to staff appropriately, with consideration for other team members' workload. j.Patients and families are satisfied with the care this nurse has given. k.Manages complex clinical situations.

2.CONTINUUM OF CARE PLANNING a.Identifies discharge/transfer needs and initiates plan for movement to next level of care. b.Uses internal & external referrals & resources that support patients/families through continuum of care. c.Initiates change in plan for next level of care in unexpected, changing and complex situations. d.Acts to resolve systems breakdowns, knowledge, and behavior issues that impact continuum of care needs.

3.PATIENT & FAMILY EDUCATION a.Incorporates a variety of resources to teach and support patient and family learning b.Individualizes patient/family education for complex patients/families c.Led in development/revision/use of teaching tools/aids for specific population within the last year.

4.PROBLEM SOLVING a.Identifies and takes action to resolve patient safety related problems and other clinical issues. b.Identifies and takes action to resolve systems (operational/workarounds) problems. c.Follows appropriate chain of command when problem solving. d.Actively participates in team/group problem solving. e.Uses interdisciplinary approach to lead others in problem solving projects and evaluates outcomes using data. f.Positively supports change. g.Is a consultant to manager (ex: clinical, proposed changes, dept enhancements).

5.COMMUNICATION & COLLABORATION a.Personal behavior positively influences environment and others (may include attendance, tardiness, team support). b.Participates in rounds with HCT c.Effectively communicates and collaborates with patients, families, and other HCT members d.Effectively communicates and collaborates with staff and other HCT members. e.Communicates positively and constructively in stressful situations. f.Provides accurate and objective feedback in a respectful and professional manner while maintaining confidentiality. g.Completes assigned evaluation tools within designated timeframe h.Receives peer feedback in a respectful and professional manner. i.Participates directly in and/or supports orientation of new staff. j.Facilitates internal and/or external customer satisfaction.

6.CONTINUOUS LEARNING a.Provide to manager evidence of self- educational activities within timeframe designated by manager. *Total # of contact hours, courses taken, articles read/hours spent in self education. b.Participates in quality improvement initiatives. c.Participates in developing and/or revising evidence based practice guidelines. d.Participates in quality improvement initiatives. e.Leads in the development and/or revision of evidence based practice guidelines. f.Participated in scholarly/professional activities in the past year
