As an Admissions Coordinator, you will:

• Contact the patient and family within 1 hour of referral, and coordinating all admission s within the 3-hour admission Commitment.
• Process referrals by obtaining sufficient admission and insurance information in order to ensure appropriate and timely admission of patients.
• Maintain communication with pending patients, their families, and referral sources and routinely follow the progress of prospective patients and those referrals not taken under care (NTUC).
• Participate in morning “Stand-Up” meetings.
• Participate as a member of the marketing team by: actively seeking new referral sources; developing and maintaining positive relationships with referral sources; assist in developing strategies and plans for Site and Company growth; participating in marketing meetings, staff meetings, department meetings, staff meetings, in-services, committees, community-education events and other activities as needed.
• Prepare marketing reports regarding referral statistics. Ensure high level of integrity of referral data within Co mpany computer system.
• Maintain working knowledge of, and assist in, accessing community resources.
• Participate in evening/weekend on-call or overtime as required.
• Perform admissions presentations in the field and via phone, as directed by ED, to assist admission team in meeting 3-hour admission standard.
• Ensure maximum reimbursement through initial payer source, benefits, and eligibility verification, authorization processes, and accurate data entry into Company computer system. Completes required payer source verification forms.

Required Skills

Qualifications

• Strong customer service experienced
• Hospice or medical office experience highly preferred
• LPN/LVN certification or Bachelor’s degree in business, social work, psychology or nursing or equivalent business experience preferred
• Able to work in an interdisciplinary setting
• Ability to maintain business and cl inical files in a systematic and orderly fashion
• Must have strong communication, organization, negotiation and public relation skills
• Proficient with the PC including the MS suite of products
