The LPN position provides nursing care in addition to assistance with daily living activities to service recipients in accordance with the Individual Support Plan and the health care plan.

Essential Duties and Responsibilities:
Ensure appropriate nursing care as specified by primary care physician
Support service recipients as specified in the Individual Support Plan
Involve servi! ce recipients in community activities per their ISP
Ensure! house is well maintained and clean
Maintain safe and positive work environment
Ensure service recipient safety at all times
Maintain appropriate communication with Registered Nurse and RSS
Minimum Requirements:
Current LPN license in good standing
Successful completion of all required DIDD training
Related experience required.
Valid Tennessee driver’s license with a good driving record
Ability to organize, keep records and meet deadlines
Starting Rate of Pay: $16.17
Hours: 7pm-7am
