The Home Health Aide is responsible for providing personal care services to patients in the home setting. The Home Health Aide provides supportive and persona! l services for patients with consideration of dignity and privacy. The Home Health Aide complies with applicable laws and regulations and adheres to Medicare and Medicaid regulations.

PRIMARY JOB DUTIES & RESPONSIBILITIES

Provides direct and indirect personal care to the patient in the home setting under direction of a Registered Nurse or Therapist.

Provides assistance with personal care, hygiene and activities of daily living as directed.

Accepts assignments and provides patient care in compliance with the Aide scope of practice and state regulation.

Utilizes safety measures in provision of care.

Maintains effective communication with supervisor regarding patient’s condition.

Participates in in-service training annually, meeting required number of hours.

Follows written assignment to deliver patient services developed by an appropriate health care professional.

Documents and submits accurate! accounts of services provided.

Observes the patient ! for changes in condition and behavior and reports to supervisor.

Removes patient's medication container from the storage area, prepares necessary items, i.e. juice, water, cups, spoons, etc., to assist the patient.

Measures and records vital signs and intake/output as assigned.

Observes for and reports any safety hazards found in the patient’s home.

Participates in case conferences and staff meetings.

Recognizes emergencies and demonstrates knowledge of emergency procedures.

Demonstrates knowledge regarding basic infection control procedures.

Reports any patient concerns/complaints or grievances to the Case Manager/Director of Nursing.

Complies with HIPAA requirements and all agency policies and procedures.

Complies with all agency policies regarding ethical practices.

Represents the agency in a positive and professional manner.

Participates in the agency’s performance/qua! lity improvement program.

Completes projects/assignments within specific timetables.

Attains goals as set forth in the annual performance evaluation.

Submits documentation within required timeframes.

Maintains strict confidentiality on patient, agency, and employee matters.

Performs other duties as assigned.

The Home Health Aide may NOT perform the following duties:
Administration of medications

Irrigation of urinary catheters, colostomies or wounds

Naso-gastric or PEG tube feedings

Catheterizations

Changing sterile dressings

Any other services not included in the patient’s plan of care

Any services requiring the skills of a licensed nurse and/or therapist

! JOB QUALIFICATIONS/PROFESSIONAL REQUIREMENTS

Education:
Graduate of an accredited high school or equivalent preferred

Training:
Must provide evidence of formal training and/or certification as a Home Health Aide as required by law. Successful completion of at least 75 hours of classroom and practical training with 16 hours of supervised practical training required.

Experience:
Minimum of 1 year full-time experience in direct patient care in an institutional setting (hospital or nursing facility), or 1 year full-time experience within the last 5 years in direct patient care in an agency setting. Home health experience preferred.

CPR per agency policy.

Current health certificate/physical examination and TB testing results (if applicable).

Evidence of adequate health status to perform described duties.

Provides evidence of competency training and evaluation as well as evidence of at! tendance at the required number of in-service education programs.

Listed on the Nurse Aide Registry with no findings against the Aide relating to patient abuse or neglect, or misappropriation of patient property.

Demonstrates satisfactory level of competency evidenced by evaluation from a Registered Nurse.

Able to follow written and verbal instructions.

Demonstrates emotional and mental maturity.

Able to safely handle body fluids and hazardous waste products.

Possesses excellent verbal and written communication skills with the ability to communicate across all levels of authority and to patients.

Maintains certification requirements as applicable to the position.

Maintains a valid driver’s license and insurance as applicable to the position.

Able to communicate effectively in English, both verbally and in writing.

Hours of Work: As required and scheduled.

Physical ! and Mental Demands & Limitations Statement: This position requires ! good physical and mental health. The position requires the physical & mental ability to stand, walk, use hands and fingers, reach, stoop, kneel, crouch, talk, write, hear and see. Demonstrates an understanding of the HIPAA (Health Information Portability and Accountability Act of 1996) reg! ulations as they apply to their job and business. Maintains strict confidentiality of all patient information, medical information and the medical record.

Discusses only appropriate patient information in public and private areas protecting the confidentiality and dignity of all patients, by actions and words.

Releases only appropriate patient information over the telephone providing for balance between guest services and confidentiality of all patients.

Maintains record confidentiality by sending all internal and external written patient information in sealed envelopes to other departments.

< >d. Has a clear understanding of the HIPAA Security rule as it relates to electronic, technical and physical safeguards for computer equipment, and access levels.
