Description: Crestview Healthcare & Rehabilitation is currently seeking to employ RNs & LPNs - FT & PRN right now, this position will be placed in Tennessee. Located located minutes from Haywood Park Community Hospital, we are solely focused on helping our patients achieve the greatest level of independence, whether they are with us for a short-term rehabilitative stay or on an extended basis.

We are currently searching for full-time Registered Nurses & Licensed Practical Nurses to join our nursing team on all shifts.

Our RNs are responsible for assessing patient health needs, developing and implementing nursing care plans, and maintaining medical records under the direction of the Director and Assistant Director of Nursing Services in accordance with the established policies and procedures of the facility, and local, state and federal regulations, to maximize the fulfillment of care-giving needs of the patients.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Provide patient care through assessments, and evaluation of healthcare plans.
Recognize and resp! ond to changes in patients conditions and document observation! s, interventions and outcomes.
Evaluate patient condition and care and develop overall care plans for patients, including rehab and restorative activities.
Review and re-write care plans as directed.
Ensure proper documentation of patient records.
Order medications from facility pharmacy.
Requisition diet orders and supplies.
Perform emergency procedures such as Cardiopulmonary resuscitation.
Recognize, intervene and report accidents and incidents when they occur.
Assist in facilitating the coordination of nursing services with other departments to maintain quality care for patients, and offer equal opportunities for employees.
Assist in reviewing, monitoring, intervention and documentation of complaints and grievances from patients, families, visitors and employees.
Assist in making routine rounds and conducting tours with others.
Monitor the workplace for possible health and safety hazards and assist in developing and conducting! routine health, safety and back care training programs to prevent worker injuries.
Attend and participate in orientation programs, on-going training and educational classes.
Maintain privacy and confidentiality of records, conditions, and other information relating to patients, employees and the facility.
Meet or exceed the dress code of the facility as specified in the Employee Handbook, which includes neat, clean, odor-free grooming and dress.
Follow the established universal precautions and isolation procedures.
Encourage an atmosphere of optimism, warmth and interest in patients personal and health care needs.

LPN Requirements:
Education: Licensed Practical Nursing License from an accredited nursing program, college or university.
Relevant Experience: Minimum of six (6) months experience in a hospital, long term care facility or other health care institution.
Computer Skills: Intermediate Microsoft Of! fice Skills.
Licenses/Certifications: Licensed Practical Nur! se license in the State of Tennessee.

RN Requirements:
Education: Registered Nursing Degree required.
Relevant Experience: Minimum of one (1) year experience implementing total patient care and care planning. Minimum of six (6) months experience in geriatric and/or psychiatric nursing.
Computer Skills: Intermediate skills, including Microsoft Office.
Licenses/Certifications: Registered Nursing License.
