Description: NM HOSPICE RN is at present looking to employ NURSE-REGISTERED 7ON 7OFF right now, this occupation will be placed in Tennessee. Screens, prioritizes, and resolves problems of patients and caregivers after-hours in order to maintain continuity of care. Works off-site from 5:00 pm to 8:00 am daily for seven (7) consecutive days; then is scheduled to not work for seven (7) consecutive days. Possesses an independent role for decision making that requires intervening with patient care matters, contacting physicians as needed, and coordinating and assigning after hours scheduled and PRN visits. Call back required. Performs other duties as assigned.

Qualifications:
Experience Minimum Required: One year experience in home care or hospice Experience Preferred/Desired : Prior case management experience in home care or hospice. Prior nursing experience as both a home health and hospice nurse. Education Minimum Required : Graduate of approved Diploma, Bachelor of Science or Associate degree in Nursing program Special Skills Minimum Required : Requires the ability to work from home for evening position, being continually available by beeper, computer and/or telephone for the designated work period. Exceptional organizational and communication skills. Licensure Description : Current Tennessee Nursing License (or license in a compact state) Licensure Minimum : RN
