This position will be responsible for administeri! ng an individualized, comprehensive and resident-centered Plan of Care in accordance with current applicable Federal, State, and local standards, guidelines and regulations as may be directed by the Administrator, Director of Clinical Services, Primary Care Nurse or Resident. Duties will include:
Utilize the nursing process to deliver quality, outcome-focused care
Develop, implement, evaluate, and revise the Plan of Care for each resident
Establish and maintain positive interactions with residents, families and other staff and serve as the primary point of contact for family members
Keep other healthcare professionals informed of residents' conditions
Ensure proper documentation to meet all reporting requirements
Promptly respond to call lights and other resident needs
Demonstrate strong ethical and safety practices
Maintain knowledge in pharmacology, lab values, and infection control
Participate in peer support / evaluation, in services,! education programs, and staff meetings, and Quality Improveme! nt activities as assigned.

Requirements:
Education: Graduate of an accredited Registered Nursing (RN) program.
Licenses/Certifications: Current state Registered Nurse (RN) license.
Experience: A minimum of one year nursing experience required. Supervisory experience with Nursing Assistants, Long Term Care and MDS/RAI experience strongly desired.
Specific Requirements
Must possess a current, unencumbered, active license to practice as an RN in this state.
Must be able to read, write, speak and understand the English language.
Must possess some computer literacy and some understanding of documentation software programs.
Must display professionalism both in appearance and attitude.
Must possess the ability to solv! e problems and make independent decisions when circumstances warrant such action.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agency personnel and the general public.
Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
Must possess the ability and the willingness to work harmoniously with and to coach and supervise other personnel.
Must be able to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures that are necessary for providing quality care.
Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult personnel.
Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
Must be able to relate information concerning a resi! dent's condition.
Must not pose a direct threat to the health or sa! fety of other individuals in the workplace.

Must be able to move intermittently throughout the workday.
Must be able to cope with the mental and emotional stress of the position.
Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
Must meet the general health requirements set forth by the policies of this facility, which may include a medical and physical examination.
Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times, hostile people within the facility.
Must be able to push, pull, move, and/or lift a minimum of 10 pounds to a minimum height of 5 f! eet and be able to push, pull, move, and/or carry such weight a minimum distance of 5 feet.
May be necessary to assist in the evacuation of residents during emergency situations.
