JOB DESCRIPTION

POSITION SUMMARY:
Provides clinical support to patients with chronic health problems through therapy compliance initiatives. Ensures collection of quality outcome data. Documents and reports patient outcomes internally. Assists patients to connect with services needed for medicine administration. Provid! es telephonic education for patients with injection questions ! and device issues. Serves as clinical resource and as a liaison between patients, pharmacy, physicians, nursing and other healthcare team members. Participates in superior customer service. Reports to and is under the direction of a designated Registered Nurse.

ESSENTIAL FUNCTIONS:

  • Coordinates patient care activities for ensuring therapy compliance by performing ongoing individual patient assessment, and medication administration coordination for chronic disease therapy.
  • Performs compliance, persistency and education call programs to assure patients adherence to therapy.
  • Participates in continuous Quality Improvement process through outcome data gathering.
  • Provides telephonic education to patients with injection questions and injection device issues.
  • Assists patients with services, such as coordination and training services for new injection devices.
  • Assist pharmacy teams when needed for special proje! cts or special requests.
  • Serves as a clinical liaison, recognizing and responding appropriately to client’s changing needs. Collaborates, communicates and confers regularly with the appropriate staff regarding patient status, provides clinical information regarding services and therapy management to patients, physicians and other healthcare professionals as directed.
  • Serves as a clinical resource and professional role-model for employees.
  • Safeguards all patient information in his/her possession and adheres to Company HIPAA Policies.
  • Provides clinical information regarding services and therapy management to patients, physicians and other healthcare professionals as directed. Conducts presentations as assigned.
  • Participates in ongoing clinical education and training. Supports manager in implementing new processes.
  • Maintains patient documentation and performs chart audits as required.
  • Provides assistance to both ! external and internal customers and ensures that company standards are ! met or exceeded. Builds and maintains internal relationships with reimbursement teams, managed care, account managers, etc. throughout the company for the coordination of patient therapy needs.
  • Develops and maintains a working knowledge of all services, products, and resources provided by company and continually incorporates new product and service knowledge.
  • Contributes to ongoing upgrade of our systems and processes as required by manager.
  • Keeps abreast of the latest developments concerning new therapies, technologies or developments reported from current nursing, medical and pharmacy literature regarding biotech drug therapy and disease management.
  • Maintains professional licensure and required certifications.
QUALIFICATIONS:
  • Graduate of accredited school of nursing, active practical nursing licensure in the state where the job is performed and 2 + years nursing experience in a clinical setting.
  • G! raduate of accredited school of nursing and 3 + years pharmacy or nursing experience in a clinical setting with client services.
  • Current licensed Practical Nurse in state of practice
  • Ability to educate others, both by phone for patients and in person when needed for staff
  • Ability to maintain confidentiality with a significant level of patient health information
  • Answer telephone queue, make outbound calls, use phone codes
  • Continually seek ways to provide better customer service both internally and externally
  • Liaison between patients, office staff, and manufacturers
  • Maintain good patient documentation within chart records
  • Objectively handle emotional situations and refer appropriately when needed
  • Strong communication skills-written and verbal
  • Knowledge in the nursing field, especially injection techniques
